Telephone Consultation Agreement
(Current as of January 27, 1999)

Thank you for your interest in our office. This office charges $100 for a telephone consultation for up to 30 minutes and $175 for up to 60 minutes. In order for our office to schedule a date and time for a telephone consultation, you will need to complete this Telephone Consultation Agreement and deposit with our office a retainer deposit. After the receipt of the Telephone Consultation Agreement and retainer deposit, you will be contacted to set a mutually agreeable time for the telephone consultatiion.

This Telephone Consultation Agreement is a modified version of our Attorney-Client Retainer Agreement. It is designed to limit the scope of our services to just the telephone consultation. Therefore, if further consultation is necessary, you will not be obligated to use our office. Similarly, if you would like our office to become your attorneys of record, then a more comprehensive Retainer Agreement must be executed.

INSTRUCTIONS: Please print this form. Read the entire form. If you agree to its terms, complete the form, sign it and mail it back to us with a retainer deposit. We cannot give any consultation without prior receipt of this executed form and a retainer deposit.

I am responsible to call Attorney at the appropriate time. Any time taken to review documents will be charged in addition to the discussion time at the above rates.

No attorney-client relationship will be formed by this telephone consultation unless otherwise agreed to in writing. Attorney is under no obligation whatsoever to represent any of the parties mentioned in the consultation. This is very important because I understand there may be time deadlines associated with the objectives sought and the benefits that may be obtained.
